You play as Isaac, a child who flees into his basement after his mother receives a divine command. Each run generates a different dungeon layout and drops a random combination of items — passive upgrades that physically transform Isaac in strange and unexpected ways. The item synergies are the heart of the game: combining the right pickups can turn a weak character into an unstoppable tear-spitting machine, while a bad combination leaves you scrambling through the later floors.
Move with WASD and shoot tears in the four cardinal directions with arrow keys. Collect heart containers, bombs, and keys; spend them in shops or use them to access secret rooms. Every floor ends with a boss fight that drops a guaranteed item. The game tracks permanent unlocks across runs — defeating new bosses and completing challenges opens additional items and characters over time.
